This visit is designed for individuals who have not connected with a healthcare provider recently. It includes a thorough review of your past medical history, current medications, and recommended preventative health screenings to help you get back on track with your care.

Episodic Visit

This service is available both in-person and virtually, and is intended for patients with an acute health concern that can be managed in a single visit. Common examples include ear infections, upper respiratory infections, urinary tract infections (UTIs), or a prescription refill.

Extended Episodic Visit

This visit is recommended for patients with more than two health concerns or for complex issues that require additional time. Examples include chronic disease management (such as COPD, diabetes, or hypertension) and mental health concerns (such as depression or anxiety).

Women's Health

Focused on women’s unique health needs, which are often under-addressed in healthcare. This visit may include breast and cervical cancer screening (if needed), contraceptive care, and discussion of menopausal hormone therapy for those approaching or experiencing menopause.

Skin Lesion Assessment

This visit includes assessment of skin lesions and, if appropriate, removal under local anesthetic with the specimen sent for biopsy. Cryotherapy is also available for lesions that are suitable for this treatment method.

Weight Management

Personalized care for patients seeking guidance with healthy and sustainable weight management. This includes a medical evaluation, lifestyle strategies, and ongoing support tailored to your individual needs and goals.

Paperwork and Forms

Support with completing medical paperwork, such as insurance forms, sick notes, or other documentation requiring a healthcare provider’s input.

Home Visit

For patients in the Blue Mountains area who prefer or require care at home. Services offered during a home visit may include health assessments, chronic disease management, or assistance with acute concerns (additional fee applies).
